To receive your mail-in rebate:

1) Purchase the specially marked Physicians Formula® Organic wear® 100% Natural
Origin Mascara product between 1/1/09 and 12/31/09.
2) Print out and mail this completed form along with the TRY ME FREE label attached
to the product and your original sales receipt with purchase price circled.
3) Rebate must be postmarked by 1/15/10.

Offer expires December 31st, 2009. Limit one rebate per household (2 rebates per
household accepted for Rhode Island residents only). Rebate amount is the
purchase price of Organic wear® 100% Natural Origin Mascara not including tax.
Maximum value is $9.95. Not valid on any other Physicians Formula product.
Requests from groups, post office boxes, or organizations will not be honored.
Fraudulent submission of multiple requests could result in federal prosecution under
U.S. Mail Fraud Statutes (18 USC, Section 1341 and 1342). Offer good in USA
only. Void where prohibited, taxed, or restricted by law. If you do not receive your
rebate within 10 weeks, visit Rebate Central or call 877-322-8355.

Awesome, I wish it was for something other than mascara as I do use Physician's Formula products, but I don't wear mascara.

I was in Walgreens today and was checking on the Mascara's and still none of them had the FREE after MIR labels, but there were some on the Mineral Wear Mineral FaceBrightener and the Mineral BronzeBrightener. Maximum value $13.95. Not sure if you use those or not, but thought I'd mention it. Also, Walgreens has a $10 Rebate on any purchase of $10 or more of Physician's Formula Organic Wear products in this months Easy Saver.

I just realized I've been looking at the Mineral Wear mascara and not the Organic Wear mascara. I have never even seen the Organic Wear mascaras in stores before. Here is a list of the 3 different rebates they have available:
